Forms for Master's Degree Students

Graduate Forms

It is important that you work closely with the graduate adviser and graduate coordinator in determining your particular needs for a given semester. Each student will have different requirements, given their unique situation. You may be responsible for the following type of forms during your studies: adding and dropping courses, registration procedures, student academic employment, degree evaluation, and convocation.

Miscellaneous Forms

Graduate Add/Drop Form, Petition for Late Registration, Authorization for Leave of Absence – please check with the Graduate Coordinator for these forms. Students must obtain approval from their graduate adviser.

Required Forms for Graduation

Please submit your Master’s Graduation Application Form online via UT Direct.

The online Master's Graduation Application Form automatically routes first to your departmental graduate adviser for approval and then to a degree evaluator in the Graduate School for final approval. The Master's Graduation Application Form is valid for one semester only and a new application must be submitted if your semester of graduation changes. If you find after filing for graduation that you will not complete your degree requirements, you may contact the Graduate School for instructions on rescheduling your graduation.

After the online deadline has passed (see Academic Calendar for semester deadline dates) you may complete the paper form below:

A web version of this form can be filled out before it is printed and submitted to Main Building 101 by the last class day.

Thesis or Report Option Students

There are currently two formats being accepted for the thesis or report: 1) an electronic version, in the form of a pdf file on compact disk (CD) or zip disk or 2) the traditionally bound version printed on cotton paper. Either format requires an additional printed copy of the following pages to be submitted separately from your thesis or report:

  • a title page
  • a signature page (with original signatures of your supervising committee as listed on your Master's Graduation Application Form)
  • and an Abstract.
Theses and reports must follow the guidelines of the graduate school format booklet, Format for the Master's Thesis and Report (.pdf). A format check for either electronic or print versions is recommended before turning in your completed work. Contact one of the following degree evaluators.

Thesis students are required to sign and submit these forms with their finished thesis and paperwork. These forms are available on the online forms page.
Failure to meet specified deadlines may result in a postponement of your graduation. Long lines to see a degree evaluator are typical around deadline days. It is highly recommended that students meet their deadlines as early as possible to avoid the added inconvenience and crowding that is expected at the last minute.
